   <title>Back Pain</title>

   <description>60 year old male from Ecuador presenting with chronic back pain for over 40 years, most recently the back pain has worsened. He describes shooting pain down both legs all the way down to posterior calf area, right worse than left, tingling (pins and needles ) mostly on the right side. Most lately at times patient can not even walk less than half of a block before stopping. He has difficulty falling sleep because of pain, And then when he wakes up in the morning the pain is still there, not as bad but always there. Pain gets worse with stress. Past medical history except for back surgery in 1990 otherwise is unremarkable, Medications; Patient says “nothing works, nothing touches the pain.” Despite the pain patient has remained active and have been working, denying disability. Patient describes himself as type B and Definitely not a perfectionist. But a real good hearted person. A type of person who can’t say no and at times bends backward and goes out of his way to accommodate others.</description>

   <description>Patient is an 83 year old male from Amish country. Patient complains of hip pain, states the whole hip has been hurting him now for almost 1 year.  Patient has a history of decompression surgery of his lower back two years ago. Otherwise his past medical history is unremarkable , he does not take medication and he still works full time in his farm. He does not smoke and does not drink Alcohol. Patient describes the pain over the anterior and posterior aspects of the right hip. Patient is reporting primary pain during prolong weight bearing activities. His X-ray studies are negative for osteoarthritis of the Right Hip.</description>

   <description>36 year old male came in today for evaluation of his left knee. Patient is concerned about preventive care and avoiding future arthritis. He states about 3 years ago while skiing he had a fall. At that time it was very painful and he needed emergency personnel to bring him down the slopes using stretcher. He had an MRI initially that showed partial tear of his ACL (Anterior Cruciate Ligament). Patient had some physical therapy and he was able to get some self healing subsequently. At this point he says he can not run (knee feels unstable), and at times if he swims a lot he gets some discomfort. He is otherwise healthy and does not take any medications.</description>

Patient states that two years ago while playing in a game he injured himself by twisting his right knee, since then his performance is not quite the same.  He really has no pain when he walks; discomfort is only during playing especially the jumping affects his game. His past medical history is unremarkable; he is very healthy and athletic. Hardly takes any anti-inflammatory, never smokes and does not drink.  Patient still has several more years on his contract, and he is concerned about preventing arthritis in future.  On physical exam twisting of the knee produces pain in the lateral region; he is also tender at the level of Lateral (medical lingo: the ligament is on the outside of the knee) collateral ligament. </description>

   <description>71 year old obese female with history of knee pain presents to the office for evaluation and treatment. Patient states she has no pain as long as she is sitting, going downstairs hurts her much more than going up. Knees are always swelled up.
Knees get real stiff after sitting for about 30 minutes. She has to use a pillow between her knees when she sleeps. She can not walk more than a few steps before stopping. 
Her past medical history is negative for Rheumatoid or any other connective tissue diseases. Tylenol helps some times. She says "I have tried everything including acupuncture , synvisc injections, cortisone injections and anti-inflammatory medication with no improvement." Patient is 5 feet tall and weights over 280 pounds.</description>

The pain gets worse when the swelling is more and when she fully extend her knee or when she walks a lot. Her past medical history except for osteoarthritis of her knee otherwise is non-contributory.  Advil definitely helps hers swelling as well as the pain. She does not smoke but she does drink socially. There is history of arthritis in the family. </description>

   <description>50 year old male who was having pain off and on in the right knee for the past 3 years is presenting with chief complaint constant sever pain the last 2 months. Patient states he had pain off and on in the knee in the past however for the last two months it has been continuous and interfering with his life style. He describes pain all the time, worse when he is walking or putting weight on it. He says the pain is still there if he is sitting but not as bad.  He is other wise healthy and only takes anti-inflammatory medicine for his discomfort.  He never had any sports injuries in the past but he describes he has been running on treadmill despite of his pain in order to stay in shape.  After reviewing his symptoms and a current MRI diagnosis of Osteoarthritis was confirmed.</description>

Her past medical history is otherwise unremarkable, she takes Ibuprofen foe her pain .She does not smoke but drinks alcohol occasionally.  She says she does not like all the sounds that the knee makes.  Discussion: The term Crepitus which is a medical term to describe the grating, crackling or popping sounds and sensations experienced under the skin and joints. The sound  can be created when two rough surfaces in the joints  come into contact , Crepitus can be heard in early cases of Osteoarthritis of the knee  where the cartilage and meniscus is starting to wear out, and the femur and tibia grind against one another.</description>

   <description>MN is a 52 year old male with history of knee pain for 15 years who visited us initially in November 2005, came in today for a consultation with me regarding his right knee. His initial complaint at that point was a severe limp. Patient past medical history is significant for the history of Arthroscopy of right knee , he is other wise healthy. Patient is looking for a possible alternative to knee replacement and further surgery at that point. On physical exam substantial tenderness mostly at the medial aspect of the right knee was noted. An Xray on the left showed massive osteoarthritis. A follow up Xray in October (on the right) limping is caused by asymmetric abnormality of the gait. Limping may be caused by pain full walking, uneven leg lengths experiencing ,muscle weakness and atrophy, disorders of proprioception, and stiff and edematous knees. Common terms for painful walking used in medicine are known as antalgic limp, or trendelenburg.</description>

There is family history of osteoarthritis.  His symptoms are specific to pain, stairs specially bothers him. He experiences stiffness after sitting more than 10 minutes. On the exam the knee is extremely bowlegged.</description>

   <description>59 year old female from Massachusetts came in with complaint of recurrent dislocation and instability of right knee for past several years.   Patient has been plagued with knee problem since a Ski accident 10 years ago. A year later she had surgery on the knee which she describes as metal plate being screwed to the knee, that was followed with another surgery a year later called Osteochondral Allograft which is very similar to doing a transplant. After the surgery she had to be on crutches for close to a year which caused her to develop Osteoporosis in the right lower extremity. After all that hard work her body rejected the transplant. Despite the unsuccessful procedure to avoid total knee replacement the surgeons attempted a second Osteochondral allograft which was followed by more non weight bearing which consequently produced more osteoporosis.  She is otherwise healthy, She takes Fosomax for her Osteoprosis , and she takes Anti-inflammatory medications if she has discomfort. Her family history is non-contributory and she is well educated with a PHD in Physical therapy.</description>

   <description>Doctors now are thinking twice about writing a prescription for the three cyclooxygenase-2 inhibitors (Vioxx, Celebrex and Bextra) because of concerns over their cardiovascular side effects. The COX-2 inhibitors were developed for patients at high risk for Gastrointestinal or stomach acidity and ulcers as well as other side effects associated with chronic treatment with traditional NSAIDs (Non steroid Anti inflammatory drugs such as Motrin, Naprosyn, Volteran, Lodine, Mobic, Relafen). Just about a year ago when Vioxx was popular (more than 60,000 death attributed to it, more than all the American lives lost in Vietnam) doctors were enthusiastic about them. And no thanks to an aggressive marketing plan by Merck it was hyped and perceived that Vioxx is more effective pain reliever than traditional NSAIDs, although this was never shown in clinical trials and was never studied. </description>

On exam the Achilles tendon is very tender, even the slightest touch or palpation makes her uncomfortable.</description>

Up to a year ago, although his range of motion was compromised but still he was able to move the neck somewhat , however since then his range of motion has been getting worse, at this point he can not move the neck at all; cervical range of motion=0 degree in all axis. He is other wise a healthy male. He takes no medications and he receives regular chiropractic treatments. He is working and is productive, and he has noted eating vegetables helps his condition.</description>

   <description>63 year old male from Mexico flew in today for evaluation and treatment of left hip, patient states 6 months ago he fell down, since then he has pain in the hip 24 hours a day. Pain interferes with his activities specially his golf game. Pain gets worse when he walks too much or lies down or roll over on the hip, the aching pain sometimes radiate down the outside of the thigh as far as the knee. Climbing the stairs, has been so painful that patient had to build an elevator for his house. Patient’s past medical history is otherwise complicated by Open heart surgery and aortic valve replacement, for which patient needs to be on Coumadin for life. His pain responds to Advil somewhat. On exam his range of motion is normal however he has excruciating pain and tenderness upon palpation of the hip, tenderness is mostly focused on the outside of the upper thigh, just over the point of the hip.</description>

Essentially unless he plays the American past time , he has no discomfort, hence no need for medications. He is some what concerned since next year is time for drafting for MLB (Major League Baseball). He has brought with him the result of MRI which shows possible tear at the level of Ulnar- Collateral ligament.</description>

Patient is status post surgical repair of a patellar tendon using a synthetic tendon this was complicated by additional tear as well as infection. Her past medical history is significant for SLE (Systemic Lupus Erythematosus ) that she has it under control with prednisone.  She does not smoke and otherwise is healthy. Patient has difficulty getting around and she has to use a walker to move around, she also states she has falls frequently due to instability of the knee.</description>

Pain started a week after his car accident which caused a whiplash, Patient has a history of fusion at level of C4-C5 ( 4th and 5th Cervical Vertebrae ). Patient describes having pain almost every day, pain radiates to the right shoulder he feels numbness at and tingling in his right thumb as well as right index finger. looking up and to the sides make him hurt worse. Patient says he can not stand for long period of times and has pain when he walks a lot . Pain exaggerate when he is under stress (when he is quarreling with the wife) and subsides when he is having a good time (being intimate with his wife). Patient states "some days it feels like the whole world is on my shoulders". His past medical history is otherwise unremarkable. He is very healthy and athletic, He only takes otc Motrin for his pain, he does not smoke or drink alcohol. There is no family history of arthritis</description>

Describes severe pain at all the time, can not walk more than a few steps before having to stop. Pain is worse at night time, not allowing him to sleep, Patient has been confined to his home mostly. His past medical history is complicated by history of Pheochromocytoma and removal of his left Adrneal gland and onset of Osteoporosis at the age of 28. Patient takes no medications, does not smoke and does not drink Alcohol. There is no history of Pheochromocytoma or MEN IIa or MEN IIb in his family. Patient denies any history of trauma.</description>
